| 二、RI Service Above Self Award申請（提名）表格，應於2012年9月1日以前寄送RI。因此， |
| 本地區有意參選者之表格務必於2012年8月28日以前e-mail到總監辦公室 |

| |
| 三、The Rotary Foundation Citation for Meritorious Service及Distinguished Service Award |
| 之提名截至期限分別為2013年6月30日及2012年11月15日，因此提名表格不必於2012年8月28日以前 |
| 寄達總監辦公室。前函所載截止日期有誤，謹予更正。 |
